If you are requesting to use our facility, please read all the information on the Facilities Usage Policy before turning in the Facility Use Request Form.
Please return the Facility Use Request Form to the church office: 13560 McLearen Rd, Oak Hill, VA 20171
Your reservation request is not final until you receive an approved copy of this form from the church office. Please do not advertise your event until you have received confirmation.
